CANCELLATIONS
If you must cancel or reschedule an appointment, we kindly request 48 hours notice. We appreciate your consideration. If we do not receive at least 24 hour notice, you may be charged for 50-100% of the service fee for the appointment.
EMERGENCIES
Our office provides on-call service for emergencies, 24 hours-a-day, 7 days-a-week. Call 973.233.1933 and Dr. Rosen, or Dr. Ablaza will be contacted immediately.
HEALTH INSURANCE PLANS
Our office does not participate directly with any health insurance plans. We have chosen this path in order to decide, with you, our patient, the best avenue of health care. It is our desire, however, to remove any obstacles which could prevent you from your desired procedure. We will help you file insurance claims with insurance carriers in providing all necessary information to insure timely and complete payment of your claim. For bill balances we accept all major credit cards.
